Q: What information do I need to give to make an appointment?
A: To make an appointment , we need your name, date of birth, address, brief description of the reason for appointment, Home and Cell phone numbers and your insurance information.
Q: What do I need to bring with me for my appointment?
A: For office visits, please bring your Insurance card, your Photo ID (drivers license), any medical records from other specialists or previous doctors including lab reports and radiology results. If you are a new patient, please print and complete the new patient forms in advance and bring them with you for your appointment.
Q: Will I get a reminder about my appointment?
A: You will get an email with your appointment date and time soon after you make it. You will receive a reminder phone call, 3 days prior to your appointment and a text message, 1 day prior to your appointment. We request you to please confirm your appointment at that time. If you need to cancel or change the appointment time please call us as soon as possible.
Q: What is your office policy if I miss my appointment?
A: If you are unable to keep the appointment, please call us ASAP – at least 24 hours in advance. If you miss your appointment and do not call to cancel, additional charges may apply.
Q: What type of payments do you accept?
A: For your convenience, we accept cash, personal checks and credit cards . You can also pay your bill online using your patient portal.
Please note if your balance is not paid within 60 days of receiving your statements, additional finance changes may apply.

A: We are available 24 hours a day. Our patients can reach us after hours by calling our Answering service at 732.678.5567.
